We Care Living provides person-centered support for adults aged 18-65, who are living with mental health conditions; who need medical treatment and interventions, but do not need to be in inpatient care; to live as independently as possible within their own homes and in the local community.

We Care Living provides 24-hour CQC Registered mental health supported living services in Southport for adults aged 18-65 who have been discharged from inpatient treatment. In addition to providing a wide range of mental health services, we assist adults in progressing along a planned path to independence.

 

We provide exemplary living facilities and bespoke support to allow our clients to live independently, happily and successfully both within their individual lives and within the wider community.

Why Choose Our Supported Living



The We Care Living care professionals work alongside the resident to identify and eliminate barriers that result in extended stays in hospital, with the aim of preventing admission/readmission into inpatient treatment. We promote integrated discharge planning which involves collaboration with hospital services, GPs, public health nurses and other professionals to adopt a client centred approach to discharge and a smooth transition into our residential care.


This also assists with a reduction in lengthy hospital stays and a prevention of readmission as service users do not remain in inpatient care longer than necessary. We provide a comprehensive range of mental health services and help adults to progress along a planned pathway to a more independent lifestyle. 


We offer personal care and encourage each individual to recover from their illness in their own time. We promote active daily living, on a daily basis our caring residential staff ensure that our clients are safe, cared for and supported. We assist our clients to work on re-establishing healthy sleep and meal schedules, self care and hygiene, medication management, healthy social and recreational activities.


Our clients also take part in a wide range of rehabilitation services such as counseling, spiritual, behavioral, physical, psychosocial and dietary.
